Step 1: Once a patient completes the Sleep Apnea Questionnaire, the dental office will receive an email notification.

Step 2: Click on the [View Patient Submission] link in the email.
This will redirect you to the patient’s chart on the Dentulu platform.
Step 3: In the patient’s chart, locate and click on the Sleep Apnea tile.
Step 4: Within the Sleep Apnea section , click on Patient Questionnaire to view the details of the completed assessment.
That’s It! You’ve now successfully accessed the questionnaire submitted by the patient for sleep apnea. With this information, you can proceed to evaluate the patient’s eligibility and plan the next steps in their treatment journey.
